When working on an issue for our benefit clients, we generally request to have information faxed to our office for review in the assistance of a claims appeal or reprocessing.

Generally we ask for a copy of the bill from the provider, an Explanation of Benefits (EOB) from the insurance carrier, your assigned member ID number, date of birth and a way we can follow up with you whether it be through a phone number or an email address.
